Compounded Medication Safety: Questions to Ask Before You Inject
A pharmacist and former pharmacy investigator examines the lessons of the New England Compounding Center tragedy, questions patients should ask about compounded sterile medications, and circumstances that may require a closer investigation.

Every time I see another advertisement for compounded peptides, weight-loss injections, or other sterile medications, I think about how much trust patients place in a product they may know very little about.
Most patients assume that if a medication was prescribed, prepared by a pharmacy, and delivered in professional-looking packaging, someone has already confirmed that it is safe. That assumption can be dangerous, or in some cases, deadly.
Compounding Has a Legitimate Purpose
Compounding is not inherently unsafe or improper. Compounded medications can serve an important medical need when an FDA-approved product is unavailable or cannot meet the needs of a particular patient. For example, a patient may require a different strength or dosage form, a customized combination of ingredients, or a preparation made without a particular dye or allergen. Compounding can also help address certain drug shortages. During the COVID-19 pandemic, for instance, some medications used for anesthesia and critical care were compounded to help meet urgent healthcare needs when commercially manufactured supplies were limited.
That distinction matters, particularly when the medication is intended to be sterile and injected into the body. A compounded medication is only as safe as the process and the people behind it. Proper ingredients, trained personnel, sterile facilities, quality controls, documentation, storage, and regulatory oversight all matter. When any part of that process fails, the patient may bear the consequences.
What the NECC Tragedy Taught Us
In 2012, contaminated steroid injections produced by the New England Compounding Center, commonly known as NECC, were linked to a nationwide outbreak of fungal meningitis and other infections.
More than 750 patients were sickened, and 64 people died, according to published CDC and Department of Justice reports. The contaminated medication was distributed to facilities across multiple states.
I assisted with the investigation involving Georgia patients. That investigation has stayed with me throughout my career, not just because of the regulatory violations, but because patients trusted that the medication being injected into their bodies had been prepared under safe and sterile conditions. They had no practical way to inspect the pharmacy, evaluate its procedures, or determine whether the product had been properly compounded, tested, stored, and transported. They depended on everyone in the medication supply chain to do what they were supposed to do.
When even one link in that chain fails, the consequences can be catastrophic.
Sterility Cannot Be Judged by Appearance
A sterile compounded medication may look perfectly normal while still presenting a risk.
The absence of visible particles, discoloration, or damage does not prove that a product is sterile. Sterile compounding requires appropriate facilities, properly trained personnel, environmental controls, cleaning procedures, ingredient sourcing, testing, documentation, storage, and shipping practices.
The FDA has repeatedly warned that patients can be seriously harmed when compounded drugs are prepared under conditions in which sterility or quality cannot be assured. A sealed vial and a professional label are not substitutes for a reliable process.
Questions Patients Should Ask
Before using a compounded injectable medication, patients should know:
Who actually compounded the medication?
The prescribing clinic may not be the facility that prepared it. Ask for the full name and location of the compounding pharmacy.
Is the pharmacy properly licensed?
Verify the pharmacy’s license with the board of pharmacy in the state where it is located. When medication is shipped across state lines, determine whether the pharmacy is authorized to ship into your state.
Why is a compounded product being recommended?
Ask whether an FDA-approved medication is available and why the compounded version is considered necessary for your specific medical needs.
What ingredient was used?
Ask about the source and form of the active ingredient, particularly with compounded peptides and weight-loss medications.
How was the product tested?
Ask whether the preparation underwent sterility, potency, endotoxin, or other appropriate testing.
How should it be stored and transported?
Improper temperature control and shipping conditions may affect the quality of a medication.
What should the label include?
The label should clearly identify the patient, medication, strength, directions, pharmacy, lot or batch information when applicable, storage requirements, and beyond-use date.
A reputable healthcare provider or pharmacy should not become defensive because a patient asks reasonable safety questions.
Compounded Does Not Automatically Mean Unsafe
The lesson is not that every compounded medication is dangerous. Many pharmacists and compounding facilities take their responsibilities seriously and follow extensive procedures intended to protect patients.
The lesson is that compounded medications require informed decision-making.
Convenience, availability, lower cost, persuasive advertising, or professional-looking packaging should never replace reasonable questions and verification. This is especially important when a product will be injected and contamination, potency problems, improper storage, or dosing errors could result in serious harm.
Do not automatically assume that a compounded medication is safe simply because someone prescribed it, sold it, or placed it in a sterile-looking vial.
Ask questions. Verify the source. Understand why the compounded product is being recommended. Then evaluate the potential benefits and risks with a qualified healthcare professional.
Trust is necessary in healthcare—but trust should never require blindness.
When Concerns Require a Closer Look
A pharmacy license search can provide useful information, but it may not answer every question.
Pharmacy-related matters may involve complex dispensing and purchasing records, interstate shipments, ownership structures, employee conduct, controlled-substance accountability, regulatory history, patient complaints, or inconsistencies between what was documented and what actually occurred.
When those concerns become part of a civil, criminal, administrative, insurance, or internal matter, an independent investigation may be necessary to identify relevant evidence, establish a timeline, interview involved parties, and clearly document the findings.
